Whanau Ora Supporting  Families and Whanau

Thank you for your interest in the Whanau Ora Service

The Whanau Ora Service is a Free, Mobile, Inter-disciplinary team funded by the Counties Manukau  District Health Board.

It is Kaupapa Maori based within a Whanau Ora framework to improve Maori health outcomes in preventable disease by delivering services to best meet peoples health need and address health inequalities. Health Priorities for this service are Obesity, Cardiovascular/Cardiac Rehab, Diabetes and Chronic Respiratory disease.

The Interdisciplinary teams consists of :

Registered nurses experienced in primary care,  and long term conditions including cardiac rehabilitation.

Whanau support worker

The purpose of the service is to:

  • Inform clients and their whanau about health risks and current disease
  • Promote lifestyle changes with smoking, nutrition and physical activity
  • Facilitate social supports such as income and housing needs.
  • Facilitate co-ordinated and integrated health care
  • Liaise with and encourage whanau utilisation of regular providers of care such as General Practice facilitating health, well being and preventing avoidable hospital admissions.
  • Promote health literacy, self management and independence.
  • Improve the health needs of Maori

Referral to the Whanau Ora Service can be from General Practice, other health professionals, community groups whanau or self.  All referrals must meet the following criteria:

  • Reside within CMDHB region
  • 16 years and over
  • At risk or current cardiovascular disease/diabetes 
  • May have a long term condition eg COPD, Asthma, CVD, Diabetes
  • Client has ability to improve and become self managing of health and well being from Nursing and/or Lifestyle intervention
  • Discussed with and consent obtained from the individual client for referral to this service

Exclusions

  • This service does not duplicate services already contracted for by the MOH, ACC or DHB for inpatient or community services (e.g. Patients receiving palliative care, dialysis)
  • Mental health/cognitive conditions and addictions are excluded.  Stable Depression can be referred.
